Ever have your email hacked but you get back into your account and change your password and security question and think everything is fine?
Well not necessarily....
When people email you they might still be getting spam through a "vacation mode" or "auto responder" setting in your email account.
Here is an example of a hacked email account that was thought of to be cleaned and fixed months ago but just discovered (only because I emailed him and I got the auto-response message reply back..) that it was sending out spam through the auto-reply vacation mode service.
Each email client has a way to check and disable it.
In this case it was in Yahoo and was under Yahoo > Options > Mail Options > Vacation Response
